VA Disability Benefits Questionnaire (DBQ)
MindBodySoul Health – Decatur, Georgia
Comprehensive Psychiatric & Mental Health Services in Decatur, GA
Veterans deserve thorough, credible documentation to support their VA disability claims. At MindBodySoul Health, our licensed mental and behavioral health clinicians complete Disability Benefits Questionnaires (DBQs) that align with the VA’s required structure. These evaluations help you clearly present how your mental health condition affects your daily functioning and quality of life.
MindBodySoul Health is not part of the Department of Veterans Affairs (VA). We provide independent clinical evaluations and documentation to support your claim. All rating decisions are made solely by the VA.
What Is a VA DBQ?
A Disability Benefits Questionnaire (DBQ) is a standardized VA form used to document a medical provider’s assessment of your service-connected or claimed condition. It helps the VA understand your current level of impairment, symptom severity, and how your condition impacts your ability to function.
When a DBQ Can Help
- Submitting initial or increased disability claims
- Providing evidence for appeals or re-evaluations
- Clarifying ongoing symptom severity
- Supporting secondary service connections or aggravation claims
A DBQ does not replace a VA Compensation & Pension (C&P) exam, but it can strengthen your evidence package.
Mental Health DBQs We Commonly Complete
Our clinicians specialize in mental and behavioral health DBQs, including:
- PTSD (Review DBQ) – for veterans already service-connected
- Major Depressive Disorder (MDD)
- Generalized Anxiety Disorder (GAD)
- Bipolar Disorder
- Adjustment Disorders
- Panic Disorder
- Trauma- and Stressor-Related Disorders
- Substance Use Disorders (SUDs) when appropriate
If your condition requires a DBQ that can only be completed by VA clinicians (for example, Initial PTSD or Traumatic Brain Injury), we will provide a clinical narrative to support your claim and help you prepare for any upcoming C&P exams.
Our Evaluation Process
Step 1: Secure Intake
Submit your information through our HIPAA-secure intake form. You’ll receive a records request checklist and instructions to verify your identity.
Step 2: Clinical Evaluation
Meet with a licensed clinician (in-person or via telehealth for eligible Georgia residents). During your session, we’ll review your medical history, current symptoms, and how your condition affects your daily functioning.
Step 3: DBQ Completion
Your clinician will complete the relevant DBQ(s), ensuring all sections reflect accurate clinical detail, diagnostic criteria, and functional limitations consistent with VA standards.
Step 4: Final Review & Delivery
Each DBQ undergoes an internal review to ensure completeness and consistency. You’ll receive a finalized PDF copy for submission through your VSO, accredited agent, or VA.gov account.
We do not submit claims on your behalf and do not provide legal representation. You may share your DBQ with your VSO, claims agent, or attorney.
